Disclaimer
Healthy Advantage (HMO)'s Preventive Care Guidelines are derived from recommendations from nationally recognized organizations. They are based on a format originally developed by the California Association of Health Plans. Since it is recognized that it is impossible to foresee all possible clinical situations, Healthy Advantage (HMO) recommends that clinical judgment be applied, and that the treatment provided to members deviate from the guidelines when individual patient considerations dictate. As with all clinical reference resources, they reflect the best understanding of the science of medicine at the time of publication, but they should be used with the clear understanding that continued research may result in new knowledge and recommendations. We recommend that the medical record contain appropriate documentation for clinical decisions.
For additional information about vaccines, vaccine supply and contraindications for immunization, please visit the National Immunization Program Website at www.cdc.gov/nip or call the National Immunization Hotline at 1-800-232-2522.
