Quality Improvement
Quality Work Plan
The University of Utah Health Plans support an active, ongoing, and comprehensive Quality Improvement Program with the overarching goal of continually monitoring and improving the quality of care and service delivered to health plan members. The Quality Improvement Program Description outlines the purpose, scope, goals, strategy, and structure of the program. The Work Plan outlines specific quality improvement initiatives.
Care Coordination
One ongoing Work Plan initiative is to improve care coordination between medical and mental health providers for their shared patients. For several years, a stakeholder group from the University of Utah Health Plans, Valley Mental Health, and the University of Utah Community Clinics has collaborated on improving communication between providers who share in the treatment of Medicaid enrollees who have mental health and medical conditions. The following documents outline how to reach providers in each respective system to coordinate care.

If you are a provider not affiliated with the groups above, or you need assistance coordinating care for a Healthy U Medicaid or University Health Care Plus enrollee, please contact our Case Management Department at (801) 587-6480, option 2.
